How can I download POA forms?

Downloading Power of Attorney (POA) Forms

1. Click Resources in the navigation
2. Click on Enrollment Forms
3. Under the Required Forms section, use the dropdown to select the state you are adding to your company
4. Click the Download button

Some POA forms require a notary or a witness to sign, depending on your state requirements.

Returning signed POA forms

1. Click Help in the navigation
2. At the bottom of the page, you’ll find a section called Contact Us
3. Click the Message Us button under Contact Us
4. Let us know which state you’re adding and that you’re sending signed POA forms
5. Upload your signed POA forms
6. Click Send
